Spoiler warning: this guide may discuss puzzles and story details. Hints stay hidden until you choose to reveal them.
This Mouthwashing walkthrough covers the puzzles, door codes, late-game Take Responsibility sequences, the single ending, five employee cards, and the optional Steam achievements players usually plan around. Wrong Organ's 2024 psychological horror story (published by CRITICAL REFLEX) is short, linear, and heavy on exploration and inventory puzzles rather than combat. Console and Game Pass availability is noted only as context - the focus here is finishing the Tulpar run and cleaning up missables.
Codes and puzzle inputs below are cross-checked from multiple independent player guides. They have not been re-verified hands-on for this article. Always confirm what your in-game Code Scanner shows on the plate or case in your build.

Spoiler and content warning
Mouthwashing is psychological horror with graphic imagery. The Steam mature-content notice from the developers warns about extreme violence, gore, and mutilation presented on low-poly characters. Store ratings and notices also flag disturbing themes. If those topics are off-limits, finish another game first - spoilers below assume you are ready to proceed.
Player guides and coverage commonly note that the story involves sexual assault, suicide or self-harm, and severe bodily injury. Read the current Steam or console store mature-content notice before you start. This guide discusses puzzle solutions and ending beats that will spoil a first playthrough.
Basics: length, structure, and tone
Wrong Organ has not published an official playtime. Review and community aggregates commonly place a first story run around two to three hours (for example RPGFan's roughly 2-4 hour note and similar HowLongToBeat-style bands). Expect closer to three hours if you read every interaction, hunt cards, or retry late chase sequences.
- Structure: linear story with a disrupted timeline presentation - one fixed ending, not branching routes
- Gameplay: first-person exploration, dialogue, inventory items, and keypad/scanner puzzles
- Combat: not a combat-heavy game; later scenes add limited gun use and chase pressure
- Platform note: PC (Steam, Sept 26, 2024); Xbox day-one Game Pass listing for Oct 29, 2026 covers Ultimate, Premium, and PC Game Pass per Xbox Wire Wave 2 - confirm your tier in the store
Controls and how to progress
Public walkthroughs describe a simple first-person kit. Exact prompts can vary by platform, so follow the on-screen UI if something differs.
- Move with WASD (or stick). Hold Shift to sprint on PC.
- Interact / use: Space or primary click (M1). Secondary click (M2) is used to aim or back out of some view modes.
- Inventory: Tab - equip the Code Scanner, axe, gun, or other story items from here.
- Locked doors and cases: equip the Code Scanner, scan the blacked-out plate or strip, then enter the revealed digits on the keypad and confirm (often the green button).
- When stuck: check the current objective text, scan nearby plates, and inspect Lounge / Medical / Utility interactables before assuming a softlock.
Sticking points and codes
The following codes appear repeatedly across independent player guides (including Steam community code lists and chapter walkthroughs). Treat them as player-guide corroborated, not hands-on verified here. If a code fails, re-scan the in-game clue - patches or UI differences can matter.
Cargo hold - code often cited as 4517
After the early post-crash stretch, the crew wants the Cargo Hold open. Retrieve the captain's Code Scanner from the cockpit area (foam-choked, but the locker / scanner pickup is still reachable per walkthroughs). Equip the scanner, reveal the digits on the plate beside the Cargo door, then enter them on the keypad.
- Commonly cited Cargo code: 4517
- Cheater achievement (Steam): open Cargo before you acquire the Code Scanner - usually a second-run trick after you already know the digits
- Always prefer the numbers your scanner shows over this page if they differ
Emergency axe / foam - code often cited as 2609
When Daisuke is trapped in foam in Utility, walkthroughs point to the Fire Axe case by the door. Scan the blacked-out strip, enter the code, take the axe, and use it on the foam (guides often say a few primary-click swings). This frees Daisuke from that foam incident; it does not prevent later story injuries.
- Commonly cited axe-case code: 2609
- Use the axe from inventory on the foam after unlocking the case
Gun case / Protection Kit - code often cited as 739
Late in the story you need the Pony Express Protection Kit (gun). Multiple guides place the scanner clue on the strip inside the cockpit locker (under the kit outline). The kit itself is commonly found in Medical - walkthroughs describe a pulled-out drawer beneath Curly's bed. Enter the three digits on the dials.
- Commonly cited Protection Kit code: 739
- Taking Notes (Steam): open the kit without returning to the cockpit first - requires already knowing 739
- Pick up Curly's employee card in Utility before you rush the gun if you are cleaning cards (see collectibles)
Locked doors in general
If a door or case will not open, look for a plate or blacked-out strip nearby, equip the Code Scanner, reveal the digits, then use the keypad. Exact button prompts and plate positions are untested here - follow the current UI.
Late-game: Take Responsibility (major spoilers)
The final stretch is spoiler-dense. Stop if you have not finished once. Solutions below combine Neoseeker-style chapter notes and other player references; valve and chase details remain marked as common player solutions, untested for this article.
Graveyard - Swansea
- Objective pressure: survive and shoot Swansea. Guides commonly say three successful shots let you proceed.
- Aim (often M2) then fire (M1). Camp a corner or narrow corridor so he approaches from one direction.
- The Good Ending is a Steam achievement, not an alternate story ending: die to Swansea ten times in this graveyard sequence (Steam description; not re-tested here).
Feeding Curly - monitor valves
In the surreal monitor sequence, rotate valves so Curly can keep the food down. A widely repeated player solution is: turn the upper valve once, the lower valve once, the right valve once, and leave the left valve unchanged. This matches common guide wording and has not been verified against the live puzzle UI here.
Ultrasound
In the vent growth scene, pick up the ultrasound from the tray, shine it on the growth, and hold primary click to focus the screen. Move until a clear four-legged / dog-like image appears, then keep tracking it until the sequence advances (guides describe the machine being taken and eyes opening).
Escape vents, IDs, Polle, cryopod
- Vent chase: keep moving and avoid the creature; screen shake/fuzz means you are too close. There is no single required maze path in public guides - survive until the scene transitions.
- Dark hallway: you cannot use the door ahead - turn around into the dark corridor of employee IDs / mouthwash piles, climb the stairs, then interact with Polle in the cargo-like space.
- Judgment / carry: pick up Curly, carry him to Utility, place him in the open working cryopod.
Ending (spoiler)
Mouthwashing has one fixed ending. There is no branching true/false route tied to cards or cocktail choices. After the birthday-table imagery, Jimmy places Curly in the remaining working cryopod, claims to have fixed things, and walks away. An off-screen gunshot follows; the final POV is Curly's as the pod activates. Finishing unlocks the Go Home achievement associated with completing the game.
Story fact vs interpretation: the cryopod placement, off-screen shot, and Curly POV are consistent across ending write-ups. Readings of the title as washing away guilt, or of birthday/cake imagery as denial, are interpretation - not official developer keys.
Five employee cards
Five employee cards are the main collectible set. Each unlocks the Steam achievement named for that crew member. Locations below are cross-checked from multiple guides. Cards are scene-tied - collect them during the listed moments. Free chapter select / post-game roaming is not verified here.
- Anya: cockpit locker, left interior wall - opening emergency-key scene (early). Grab it before you leave.
- Swansea: broken Lounge vending machine, lower dispensing tray - during the axe-search scene.
- Daisuke: Lounge coffee table beside the board game in front of the sofa - "Check On Curly" scene; pick it up before checking on Curly.
- Jimmy: inside the hat by the broken Polle statue in the Lounge - cocktail / mocktail scene.
- Curly: Utility floor beside the right-hand (working) cryopod - collect before taking the gun.
Achievements worth planning
Steam lists 18 achievements. Some ride along with story progress; others need deliberate cleanup. Do not treat achievement names as alternate endings.
- Five crew cards: Anya, Swansea, Daisuke, Jimmy, Curly (locations above)
- The Good Ending: die to Swansea 10 times in the graveyard (achievement only)
- Yimpy: maintain eye contact for 10 seconds with the Yimpy drawing - player guides place it on the Medical corkboard; exact chapter number and whether zoom is required remain uncertain
- The Name of the Game: Steam text says use a bottle of Mouthwash (in-game unlock quirks unverified here)
- Wasteful: mix Pony Express's special mocktail for Curly's birthday party (Steam text; birthday drink is separate from any spiked Utility mocktail recipes in player guides)
- Sock Puppet: Steam text describes repeatedly opening and closing Curly's mouth
- Cheater / Taking Notes: Cargo before scanner; Protection Kit without revisiting cockpit (see codes)
Only unlock conditions backed by Steam text or multi-guide agreement are listed. Do not invent extra requirements (for example unsupported cake-order steps for Wasteful).
FAQ
How long is Mouthwashing?
About two to three hours for a first story run per review/community aggregates - not an official developer figure. Card and achievement cleanup adds another pass.
Are there multiple endings?
No. One fixed ending. The Good Ending is an optional achievement in the graveyard, not a second story finale.
Is there combat?
Most of the game is exploration, reading, and puzzles. Late scenes add limited shooting and chase segments. Difficulty is personal; this guide does not promise every action beat is easy.
Where do players get stuck?
- Forgot to scan a door/case plate with the Code Scanner
- Cargo 4517 / axe 2609 / gun 739 not matching - re-scan in your build
- Graveyard: reposition to a corridor and wait for a clear shot
- Feeding valves: try the common upper/lower/right-once, left-unchanged order
- Ultrasound: keep the clear four-legged silhouette centered until the scene advances
- Vent chase: prioritize distance from the creature over finding a perfect route
